Campus Chapter Reporting

Chapter leaders are responsible for making sure that the following forms are complete. Please contact sarah@nssrh.org if you have any questions.

Please refer to the copies of the forms on our website so that you are prepared to answer all of the questions in the forms, and consider preparing them in advance of the deadline.

The reporting requirements are as follows:

Timelines, grace periods, and deadlines
For reports due quarterly, please have the report filed electronically by the 15th of the month (Q1: December, Q2: April, Q3: June, Q4: August).

These include the Member Report Form and the Event Report Form. A grace period of one semester will be granted, during which NSSRH will contact you to remind you of your reporting obligation. If the report is not filled within five business days of the reporting deadline, financial support from NSSRH national will be withheld until the report is received. Financial support will be reinstated when reporting is brought up to date.

For reports that are due annually, please have reports filed electronically within 14 business days of the last day of the academic year, as listed on your school’s website. A grace period of 30 days will be granted, during which time NSSRH will contact you to remind you of your reporting obligation. If the report is not filled within 30 days of the last day of the academic year, financial support from NSSRH national will be withheld until the report is received. Financial support will be reinstated when reporting is brought up to date.

Extenuating circumstances: We are aware that the beginning and end of semesters and academic years are often hectic times of transition, and that consistency in leadership can be variable over time. Please contact us regarding reporting if there is a reason that your chapter was unable to fulfill the reporting requirements, for these or any other reason. Our goal is to provide continuous support to your chapter, and good communication about your chapter’s circumstances will empower us to do this. A general accounting of scholarships and grants applied for and the outcome, a general accounting of expenses, due annually.
Due annually
An accounting of any events held, including organizers and participants, monies spent or collected, description of the event, reporting forms collected ie: surveys or pre-post questionnaires, event follow up plans, due at the end of every semester.
Names and contact information of membership and event attendees (sign up sheets) due at the end of every semester. Please upload your Member and Outreach Sign-up Sheet here.

Please include the following information:

~Chapter name
~Chapter leader name, email, phone
~Please enter names, contact information, and status for all persons who attended meetings and events this quarter

It would be nice if these are fields that you can increase as you need them for manual entry, but also if leaders can import from an excel sheet or other electronic sign up sheet that has the same format....making this EASY is the goal.
